MDS Coordinators Will- Direct the Residents Assessment Process through completion of the Minimum Data set (MDS) and Care Area Assessments (CAA's) Lead the development of individualized resident Care Plans Write restorative programs and monitor outcomes Identify the needs of the resident and projected outcomes; as required by Federal and State Regulations.
Requirements Must possess a valid IL.
RN or LPN License.
The ideal candidate will have at least 2 years previous experience with both PPS and OBRA assessments.
Strong understanding of the Medicare and Medicaid Reimbursement System is preferred.
General knowledge of rehabilitative and restorative nursing, laws and regulations governing Long-term Care, computer proficiency, ability to manage deadlines, and strong analytic skills.
Pay is very competitive and commensurate upon experience and education.
